CALLING ALL COACHES/MANAGERS



The Corner Brook Minor Hockey Association is always looking for eager volunteers to give of their time to help out the kids by Coaching. The following are a list of forms that will need to be completed to be able to function as a coach under Hockey Newfoundland and Hockey Canada.

HNL Screening Form

Coach Application (REP TEAM)

Volunteer / House League Coach Application

Court Check

Certificate of Conduct Fee Waiver Letter (has to be signed by registrar)

RNC Records Screening

RNC Vunerable Sector Screening

RCMP Records Screening (with Vunerable Sector screening)



THE PROCESS:

If you haven't already done so, complete the HNL Screening form.  Complete the coaching application REP TEAM or HOUSE LEAGUE/VOLUNTEER form.

Submit your completed applications to the registrar by:
  • Dropping the completed forms off at The Minor Hockey Room
  • Scanning the completed forms and e-mailing them to the REGISTRAR.
A Certificate of Conduct  is required annually for each Coach, Manager,Trainer  or Volunteer dealing with kids in ANY capacity. 

The following is the process to obtain your certificate of conduct.
  1. Obtain Fee Waiver Form signed by the Registrar (PRIOR TO AUG 9, 2010, SIGNED COPIES CAN BE PICKED UP AT THE MINOR HOCKEY ROOM. PLEASE CALL FIRST @ 639-8888)  Complete the Court Check form and submit both completed forms to The Provincial Court. Once this check is complete you will be notified to pick up you court check.
  2. Complete either the RNC Records Screening Form and Vunerable Sector Form or the RCMP Records Screening Form (Vunerable Sector included). If you live in an are patrolled by the RNC then follow this route, if your area is patrolled by the RCMP then follow this route.
  3. Bring your completed Police Screening forms AND your Court Check to the local Police Detachment for processing.

In addition, ALL coaches or on ice volunteers are expected to have completed or be willing to complete the Hockey Canada "SPEAK OUT" Course. These courses are offered through Hockey Newfoundland's web site and regularly offered through Corner Brook Minor Hockey Association.
